Chronic Disease clinics

We provide nurse-led asthma, COPD, diabetes and CHD clinics.  All patients with these conditions will be invited to an annual review with the relevant nurse.  Please endeavour to attend these appointments as it enables you to manage your condition more effectively with support and guidance as appropriate.

Asthma & COPD - Tuesday and Wednesday afternoons with Nurse Jennie

Diabetes Clinics - Tuesday morning with Nurse Heidi.

CHD clinics - Thursday afternoon with Nurse Nicki.
