New Patients – Registration Process
To register as a new patient please pick up a registration form from the surgery, or click on the links within the registration pack to download the forms. Please complete the forms and bring it with you on your first visit.
Please also complete the medical questionnaire. This is because it can take a considerable amount of time for us to receive your medical records. There is an on-line version of this file within the Registration Pack, which you may fill out and bring in. At the point of registration you will be offered an initial check appointment and a provided with an information pack. You can, of course, download all the information from the Registration Pack if you wish to peruse prior to registration.
Change of Address
Proof of your new address will be required in the form of an up to date utility bill, bank statement or other document (e.g. tenancy agreement or benefits agency letter) which shows your name and address.
Change of Personal Details
When changing a name a copy of your Marriage Certificate or Deed Poll will need to be provided.
Nicknames and short versions of names cannot be used on our clinical system. Your name should appear as it does on your birth certificate or deed poll document.
If your date of birth is incorrect on your records, we will require proof via either your passport or birth certificate before we can make any changes.
